extrude doesn't work at 26.58

Question

I'm trying to extrude to the 3D curser and it doesn't work, instead it extrude somewhere else. I have selected pivot point to 3D curser and followed all the steps. When I'm trying to do it, it doesn't lock to Y axis. It's trying to go to the 3D curser but everything gets glitched and the shape deforms when the mouse reach halfway to the 3d curser. Can you help. Thanks

I did press Shift+Y but it still does it, do i have to apply the shrink wrap modifier before i do this?

I just filled in the face to continue on to the next video but i had to use extrude there as well so i just applied the shrink wrap and it was working fine but im not sure if i was meant to apply it.

When I try to extrude like this to the 3D curser, this happens. I pressed "E" then "S" and "SHIFT" "Y". Do I have to apply the shrink Wrap first before doing this?
