2nd question... I noticed if I go into the flow tab it shows as smoke even though when I created the domain I selected fire + smoke. If I try to change it to anything other then smoke and go back to the domain tab it immediately reverts to smoke, i.e. the change in settings aren't sticking. The same goes for the collision tab. Trying to run the simulation from any other tab then the Domain tab and no simulation, so it appears the other tabs are currently useless. Is this a bug in the current version of Blender?

    Ends up I was thinking of the domain object and flow object as a matched pair. With this in mind I didn't realize that the domain tab was for the domain object and the flow tab was for the monkey, i.e. I needed to select the monkey then select the flow tab.... all is working now that I realized this. Not sure if it was mentioned but I missed it.

    I think the confusion came in when we used the quick fire without creating the individual domain and flow objects. Since they were created at the same time, they became 'joined at the hip' in my mental reference.

    I had not thought of it that way before, but it does make sense that one might assume they are connected. Glad you got it worked out!
