What if I want to twist the arm so that the palm of the hand is facing outwards? I played around with weight painting a lot, but I was unable to figure out how to do it without creating severe twisting in either the wrist or the elbow. Looking at my arm in real life, I notice that the twist is very subtle near the elbow, and closer to the hand it's a lot more, as my hand is rotating 180 degrees. Is there a way to create this gradual rotation with weight painting? Or would I need more arm bones to simulate it?

Forearm twisting is one of the trickier tasks for a rigger. It's certainly doable - Rigify handles it pretty well. However the biggest problem is fixing the "baloon twisting" effect. That would take corrective shape keys. At least that's how I'd do it.
