Sorry if this is a naive question, but are you using a digital pen (Wacom, Livescribe, etc..) of some sort with Photoshop, or are you doing this with the mouse?
This is done with a Wacom Intuos 3 tablet, I would not recommend doing concept or illustration art with a mouse. Much easier with a tablet of some kind =]
