When you change the size of the brush and the strength, I see an image of the texture you're painting with inside the circle. Is that an add-on, a setting, or something else? Using 2.77 and I just have a grey circle (with appropriate strength/size at least).

The image is representative of an brush texture being assigned to the brush. It won't appear if you don't have a texture assigned.
