So I think at some point I might have done an accidental merge at center but whats weird is that is it only appearing in Object Mode. When I go to Edit mode, the distortion isn't there. When I tried creating it into a mesh, the distortion then appears in Object Mode.

    Hm,

    Are you aware, that Mesh components (Vertices, Edges, Faces) that are Hidden in Edit Mode, still show in Object Mode. Maybe it's something like that?

    Yeah that would be my guess as well. ALT + H to bring back the hidden elements and see if any comes back.

    What they said or possibly a modifier may be causing it. 

  • Yeah hiding didn't seem to fix it. I ended up turning it into a mesh, deleting the center vertice and just refilling the empty spaces which thankfully was only one.

    My guess would be it was the clamp overlap on the bevel modifier. It's been doing weird stuff like that in the latest versions, sometimes for no obvious reasons. And since you have it deactivated in edit mode, but active in object mode, you'd only see it in object mode.
