Without seeing anything, I'd say that maybe you have 'Double Vertices'...in Edit Mode: try pressing A (to select everything), M > Merge > By Distance.
Screenshots is fine but it's even better if you can record a screen grab. You also may be going over with the inset until it crumples on itself.
