Mesh Issues

Hello! I keep having the same problem with the mesh and I don't know when it occurs and how to avoid it. Smooth/Fill brush help only untill I add clay. Thanks!

Maybe check to see if you have a locked axis:

I get the same issue when sculpting ears, it's like the problem appears if I try to sculpt something thin

See if you have Front Faces Only enabled:

When sculpting on a thin mesh, that can be helpful, but it can also push the front through the back.

It seems like I  resolved the issue by lowering dyntopo resolution to 20, adding and then substracting clay

    Thanks for the help!

No, you don't have to start over because of that. There is a technique Kent uses whenever I see a sculpting course of his. When he gets into that sort of problem, he first makes a swift and soft pass with the inflate brush, with low strength over the surface, just to accommodate the polygons, because there might not be enough resolution over that area. Then when you sculpt normally again, now that there is enough polygons in the surface, it doesn't behave strangely.

You can also upload your file for us to take a look if you want.
