You can't see this in Face Select Mode, but I suspect that you have Double Vertices; maybe you Inset once and then you tried again to Inset with Outset enabled, but that immediately hits the outer rings that were created with the first Inset.
That's a lot of words, clumsily trying to explain this...
Just select all and Merge by Distance.
If that doesn't work, maybe provide a link to your .blend
Yes you can tell you have doubles because the edges are not highlighted. Here with the vertices selected, you can see how the edges connected to the vertices get that orange selection highlight.
And here you can see how the edges are not highlighted because the vertices are doubled up.
thank you I just tried it and there were 8 vertices removed and outset worked, lol that will teach me to stop blender work after 11pm
