When using smooth feature it shrinks the fins fast. I have tried the settings in smooth brush but still lose a lot of topology. Both radius and strength turned down dont seem to just smooth the edges and are just eating the mesh away. Running blender 2.93.6. Also is there anyway to increase the mask color, Mine is kinda a medium grey and not anywhere near as black as the video. I have reset blender to factory settings just incase a add on was causing a issue but it actually worse.
are you working with a mouse or do you have a tablet/pen with pressure sensitivity?
another thing to know is if your brush size is large enough to smooth both sides of your mesh at the same time...
Im currently working with a mouse. I do have a tablet but I am not really good at using it. I still cant get a straight line out of it. Could the tablet be controlling the pressure even when not in use.

It shouldn't!
The Masking color is an Overlay and it is set to 0.75 as default...try changing that..also make sure the Strength of the Mask Brush is 1:
If you have a lot of topology detail (read many Vertices), then the Smooth Brush won"t affect the Mesh so much, but if you try to smooth the default Cube (with only 8 Verts), the smoothing will be "catastrophical"...
Thank you for the Mask Overlay setting. That did the trick. I did not know that was there. In the video it shows only mask the area, pull out the fins, Remesh with .02 voxels, and with the grab brush morph to wanted style and hold shift to Smooth. The video smoothing barely removes topology but every time I try it its removing or shrinking the are drastically. If there isnt enough Topology after the remesh then the voxel size could be wrong. Still really new to this so trying to figure out if i did something different.
Why do I have 2.93.1 version while you have 2.93.5? It must be the beta thing because I had the alpha version.
