Any ideas on how I could do the mouth and eye ?

I'm obviously going to render diffrent passes although at first I wanted to avoid doing that in order to keep the smoke really in the bottle (and not fake it comp) but the more I get into this and the more I feel like I'll have no choice. ^^'
I first read your post as meaning you wanted the teeth as smoke. I would model the teeth and eye or grab a model off the internet. Teeth can be tough depending how real you are after. There are some good blender studies on teeth out there. Subsurface scatter, translucency, refraction, lighting....
In the compositor you can adjust how much of an overlay or blend you want with the smoke.
I would use the compositor much like you would use After Effects. You can render the smoke and objects as separate passes allowing you to manipulate them individually without having to render the image.
It will just be a still yes. The smoke is already done (that's what you see) what I want is to add the mouth and eye on top.
The pb that I have is to to get a way to achieve the look of "interior" in the mouth and volume of the eye whilst still keeping the original form. (quite dificult to explain)...

Cheshire cat effect. If you get the blend of all these subtle effects to play nice it will be great.

Enough time and patience you could use a shaped emitter and capture the right frame then use the compositor.
