Well I was try to Inset Faces, I see two individual faces seperate insert where am I to wrong. I was try a merge vertex it's not work.

The only thing I can think of, is that you have double Vertices (as you suspected). Go back to before the Inset, Make sure to Select all the Verices between those Faces (Wireframe Mode) and Merge By Distance. Increase the Distance if necessary.

I have some problem on bevel in same area. i think have a to much edge up to up 4 or 5 maybe. I'm not sure is it the problem but I have done one way or other. :)
